Tratamiento fisioterapéutico tras reconstrucción del ligamento cruzado anterior: seguimiento de dos casos clínicos.

Abstract

El objetivo principal de este trabajo es describir la evolución de dos pacientes operados de lesión de ligamento cruzado anterior tras seguir los protocolos de tratamiento del Hospital Ramón y Cajal, y determinar si podrían ser mejorados para conseguir una recuperación más rápida sin dañar la articulación. Los objetivos secundarios se basan en indicar todos los aspectos sobre este ligamento y su recuperación que el fisioterapeuta debe conocer para proporcionar una rehabilitación adecuada al paciente lesionado. Para ello, se aplicaron las pautas establecidas por los citados protocolos y se hizo un seguimiento de ambos pacientes desde el inicio de su rehabilitación. Posteriormente se realizó una revisión bibliográfica de 24 artículos y siete libros, empleando para su búsqueda las bases de datos MEDLINE, PEDro y Cochrane, así como bibliotecas de Ciencias de la Salud. Tras la finalización de los periodos de tratamiento, el paciente que siguió un protocolo más acelerado obtuvo un mayor éxito en su recuperación. En base a los resultados obtenidos y a la evidencia científica, se puede concluir que es necesario modificar los protocolos empleados en el Hospital, teniendo siempre en cuenta que debe primar el estado y respuesta de cada paciente frente a las pautas establecidas por dichos protocolos, pero que éstas son necesarias para asegurar el éxito de la rehabilitación.

The main goal of this project is to describe the evolution of two patients undergoing anterior cruciate ligament reconstruction, after following the treatment protocols of the Ramón y Cajal´s Hospital, and determine whether they could be improved to achieve a faster recovery without damaging the joint. Secondary objectives are based on point all aspects of this ligament and its recovery that the physiotherapist should know in order to provide adequate rehabilitation to the injured patient. To this end, the guidelines stablished by those protocols were applied and a tracking of both patients was done from the start of their rehabilitation periods. Subsequently a literature review of 24 articles and seven books was performed using the MEDLINE, PEDro and Cochrane databases, and Health Sciences libraries. After the completion of the periods of treatment, the patient who followed the faster protocol was more succesful in his recovery. Based on the findings and evidence, it can be concluded that modifications in the protocols used in the Hospital are necessary, taking into account that the state and response of each patient must take precedence to the guidelines established by the protocols, but they are necessary to ensure the success of rehabilitation.
