Abstract
INTRODUCCIÓN
Las guías de las sociedades de Gastroenterología pediátrica de Europa y Estados Unidos, PSGHAN/NAPSGHAN, establecen las recomendaciones necesarias para el adecuado manejo de la infección por Helicobacter pylori (H. pylori) en la población infantil. Parece existir dificultad en su correcto cumplimiento, lo que, unido a las altas tasas de resistencia a los antibióticos, resulta en un porcentaje de erradicación muy alejado del objetivo propuesto. Este estudio analizará las resistencias antibióticas de H. pylori en la población infantil de Guadalajara, así como la adecuación del manejo de la infección a dichas guías.
MATERIAL Y MÉTODOS
Se ha llevado a cabo un estudio transversal, descriptivo y retrospectivo en la población pediátrica del Hospital Universitario de Guadalajara. Se han analizado 98 pacientes valorados en la consulta de Gastroenterología infantil, remitidos desde Atención Primaria
por detección de infección por H. pylori, en el periodo comprendido entre el 1 de enero de 2015 y el 31 de diciembre de 2019.
RESULTADOS
Se realizó test no invasivo desde Atención Primaria en 83,8% pacientes. El 23% recibió tratamiento empírico, siendo OCA la pauta elegida en el 91,3% de los casos, obteniendo una tasa de erradicación del 3%. La tasa de resistencia al metronidazol ha sido del 27,8%, a la claritromicina del 22,2%, y no se detectaron resistencias frente amoxicilina, levofloxacino ni tetraciclina. La tasa de erradicación final ha sido del 51,7%. Se han encontrado diferencias estadísticamente significativas en las tasas de erradicación en los pacientes en los que se llevó a cabo endoscopia y el tratamiento fue dirigido por antibiograma (p<0,001).
CONCLUSIONES
La estrategia “test and treat”, desaconsejada en las guías de consenso para el manejo de la infección por H. pylori en niños, sigue siendo empleada en nuestro medio desde Atención Primaria. Las altas tasas de resistencia para claritromicina y metronidazol
halladas en este trabajo, representan el patrón constante descrito en las guías de nuestro entorno. Los resultados de este estudio muestran la necesidad de basar el tratamiento en el antibiograma para alcanzar el 90% de erradicación deseado. INTRODUCTION
The guidelines of the pediatric Gastroenterology societies of Europe and United States, EPSGHAN / NAPSGHAN, establish the essentials recommendations for an appropriated
management of the Helicobacter pylori (H. pylori) infection in children. It seems like there are troubles in their suitable compliance, which, together with the high resistance antibiotics rates, result in an eradication’s percentage out of the proposed objective. This study will analyze the antibiotic resistance of H. pylori at Guadalajara´s children
population, and the adaptation to the management of the infection to the mentioned guidelines.
MATERIAL AND METHODS
A cross-sectional, descriptive and retrospective study, has been carried out in the pediatric population at the University Hospital of Guadalajara. A total of 98 patients referred from Primary Health Care for detection of H. pylori infection, have been analyzed after being assessed at the pediatric Gastroenterology consultation, during the period from 1st January 2015 to 31th December 2019.
RESULTS
Noninvasive tests have been carried out from Primary Health Care in 83.8% patients.
23% received empiric treatment, and OCA was the chosen option in 91.3% patients. The eradication rate was 3%. The resistance rate to metronidazole has been 27.8%, to clarithromycin 22.2%, and no resistance has been detected against amoxicillin, levofloxacin, or tetracycline. The final eradication rate has been 51.7%. It has been found
statistically significant differences in eradication rates in patients who underwent endoscopy and treatment was individualized considering antibiotic susceptibility (p <0.001).
CONCLUSIONS
The consensus guidelines advise against “test and treat” strategy for the management of H. pylori infection in children. It continues being a strategy used in our setting from Primary Health Care. The upper resistance rates to clarithromycin and metronidazole
found in this work, reflect a constant pattern described in our environment guidelines.
The results of this study show the need to choose the treatment according to antibiotic susceptibility to achieve the 90% eradication rates desired.
