Abstract
Abstract: after the first year of postpartum, 36% of women present diastasis recti
abdominis (DRA), condition that affects the quality of life after childbirth. There is also
a higher risk in this period of time of developing pelvic floor dysfunctions (PFD).
Nowadays, the use of unestable plataforms, like the 5P® LOGSURF, are being used
to treat PFD and given the existing muscular synergies of the pelvic floor muscles
(PFM) and abdominal muscles, some studies point to the use in treatment of DRA.
Objectives: describe the distance interrectus (DIR) in supine decubitus, standing on
5P® LOGSURF while resting, with contraction of the transversus abdominis muscles
(TrA) and contraction of the PFM.
Methodology: a transversal observational study is proposed. The subjects of the
study are women of legal age, primiparous in postpartum that have freely read,
understood and signed the informed consent. The DIR, muscular thickness of the
TrA muscles and the rectus abdominis muscles (RA) will be measured by ultrasound,
as well as the basal tone and the PFM strength by intravaginal dynamometry in
supine position, and standing with and without 5P® LOGSURF. To evaluate the
practicability and acceptance of the proposed study, a pilot test will be conducted
with the purpose of identifing: 1) the recruitment rates; 2) the relevance of the
inclusion and exclusion criteria; 3) the reason of abandonment, withdrawal and
rejection; 4) the size of the effect and variability to calculate the sample size; 5)
strengths and limitations, as well as proving: 1) the variability of the procedure and 2)
the acceptability of the measures of results.
Results: the pilot test was feasible and achievable for the objectives proposed. The
recruitment was conducted by a private clinic of Madrid, being insufficient for the 57
participants needed according to the sample size; so other recruitment measures
were required. The inclusion and exclusion criteria were consistent with the studied
poblation with the exception of cesarean delivery. The acceptance of the procedure
was of a 86% and the rejection had to do with the lack of knowledge of the use of
intracavity techniques as a part of the process. The procedure was viable and the
measures of results acceptable for the DIR variables and abdominal muscles
thickness, as well as the PFM in supine position, and standing with and without 5P® LOGSURF. The measures of results obtained showed 1) the infra-umbilical DIR
which obtained its lowest value on 5P® LOGSURF with maxim voluntary contraction
of the PFM, 2) the supra-umbilical DIR obtained its lowest value in supine position ,
3) the increase of factors regarding the PFM and 4) the thickness of the TrA and RA
muscles on 5P® LOGSURF comparing the results gathered while standing and
supine position.
Conclussions: the pilot test demonstrated feasibility and acceptability to evaluate
the DIR, abdominal muscles and PFM in supine position, and standing with and
without 5P® LOGSURF. The improvements that should take place before conducting
a complete study are expanding the recruitment and explication of the procedure
prior to the measures of results, as well as reviewing the inclusion and exclusion
criteria. Regarding the effect of the possition and muscular recruitment, a difference
in DIR IN and DIR SU was observed. While the DIR SU presents a bigger closure in
supine possition, the DIR IN presents it on 5P® LOGSURF. The PFM presents a
higher tone and strength on 5P® LOGSURF and the thickness of the TrA muscles is
also higher on 5P® LOGSURF. Introducción: tras el primer año de postparto, el 36% de las mujeres presenta
diástasis del músculo recto abdominal (DRA), condición que afecta a la calidad de vida
de las mujeres puérperas. Esta etapa también supone un riesgo para padecer
disfunciones del suelo pélvico (DSP). Actualmente, se está introduciendo el uso de
plataformas inestables, como el 5P® LOGSURF, para el manejo terapéutico de las
DSP y, dadas las sinergias musculares existentes entre la musculatura del suelo
pélvico (MSP) y la musculatura abdominal, algunos estudios sugieren que podrían
tener utilidad para el tratamiento de la DRA.
Objetivos: describir la distancia interrectos (DIR) en decúbito supino, bipedestación y
bipedestación sobre 5P® LOGSURF en reposo, con contracción del músculo
transverso del abdomen (TrA) y con contracción de la MSP.
Metodología: se propone un estudio observacional transversal. Los sujetos de
estudio serán mujeresmayores de edad, primíparas que se encuentren en el postparto
y que hayan leído, comprendido y firmado libremente el consentimiento informado. Se
recogerán la DIR y el grosor muscular de los músculos TrA y recto abdominal (RA)
mediante ecografía, y el tono basal y la fuerza de la MSP mediante dinamometría
intravaginal en posición supina, bipedestación estable y bipedestación sobre 5P®
LOGSURF. Para evaluar la factibilidad y aceptabilidad del estudio propuesto, se
realizará una prueba piloto con el propósito de identificar: 1) las tasas de
reclutamiento; 2) la pertinencia de los criterios de inclusión y exclusión; 3) los motivos
de rechazo, retiro y abandono; 4) el tamaño del efecto y su variabilidad para el cálculo
del tamaño muestral; 5) fortalezas y limitaciones; así como probar: 1) la viabilidad del
procedimiento y 2) la aceptabilidad de la toma de medidas de resultado.
Resultados: la prueba piloto fue factible y aceptable para los objetivos planteados. El
reclutamiento se realizó en una clínica privada de Madrid, lo cual resultaría insuficiente
para las 57 participantes necesarias según el cálculo del tamaño muestral; siendo
necesarias otras modalidades de reclutamiento. Los criterios de inclusión y exclusión
fueron congruentes con la población que se pretendía estudiar a excepción de la
cesárea. La aceptabilidad del procedimiento fue del 86% y los rechazos tuvieron como
motivo el desconocimiento del uso de técnicas intracavitarias como parte del procedimiento. El procedimiento fue viable y la toma de medidas de resultado
aceptable para las variables DIR y grosor de los músculos abdominales, así como de
la MSP en supino, bipedestación estable y bipedestación sobre 5P® LOGSURF. Las
variables resultado obtenidas mostraron 1) La DIR infraumbilical (IN) obtuvo su menor
valor sobre 5P® LOGSURF con contracción máxima voluntaria de la MSP, 2) la DIR
supraumbilical (SU) obtuvo su menor valor en posición supina, 3) aumento de las
variables relacionadas con la MSP y 4) el grosor de los músculos TrA y RA sobre el
5P® LOGSURF obtuvieron su mayor valor frente a las recogidas en bipedestación
estable y en supino.
