Abstract
Las lesiones discovertebrales entre las tropas españolas desplegadas en Zona de
Operaciones (ZO) constituyen una causa cada vez más frecuente de evacuación
estratégica a territorio nacional (TN). Asimismo, suponen un importante motivo de
incapacidad e imposibilidad para el desempeño de la misión, siendo una de las
principales causas de pérdida de personal militar.
En este estudio retrospectivo se han revisado un total de 28 historias clínicas de
pacientes militares evacuados por patología vertebral desde el Teatro de Operaciones al
Hospital Central de la Defensa “Gómez Ulla” (HCDGU), realizadas entre los años 2009
y 2020. Las operaciones en curso de las Fuerzas Armadas Españolas que generaron el
mayor volumen de bajas por estas lesiones fueron las navegaciones de los distintos
buques de la Armada Española, principalmente del mar Mediterráneo (EUNAVFOR
MED – Operación “SOPHIA”), seguido de la misión en Afganistán (ISAF).
Del total de las evacuaciones efectuadas, el 43% fueron por causa accidental y el
39%, por reagudización de la patología vertebral previa. La columna lumbar fue la
región más afectada, siendo las fracturas el diagnóstico traumatológico principal (37%).
La radiografía convencional constituyó la prueba complementaria más utilizada
para el diagnóstico. La mitad de los pacientes (n= 14) fueron intervenidos
quirúrgicamente: cuatro en ZO, y diez tras la evacuación a TN, de los cuales sólo uno
había sido operado durante el despliegue.
El aumento del número de las lesiones discovertebrales unido a la complejidad
de su tratamiento traduce la importancia para la Sanidad Militar española de promover y
mantener el Hospital Central de la Defensa “Gómez Ulla” como elemento indispensable
en el apoyo médico a las misiones internacionales Discovertebral injuries among Spanish troops deployed in the Area of
Operations (AO) are an increasingly frequent cause of strategic evacuation to National
Territory (NT). They are also a major cause of incapacity and inability to perform the
mission, being one of the main causes of loss of military personnel.
In this retrospective study, a total of 28 medical records of military patients
evacuated due to spinal pathology from the Theatre of Operations (TO) to the Defense
Central Hospital "Gómez Ulla" (DCHGU), carried out between 2009 and 2020, were
reviewed. The ongoing operations of the Spanish Armed Forces (SAF) that generated
the highest volume of casualties due to these injuries were the sailings of the different
ships of the Spanish Navy, mainly in the Mediterranean Sea (EUNAVFOR MED -
Operation "SOPHIA"), followed by the mission in Afghanistan (ISAF).
Of the total number of evacuations carried out, 43% were due to accidental
causes and 39% due to exacerbation of previous spinal disease. The lumbar spine was
the most affected, with fractures being the main trauma diagnosis (37%).
Conventional radiography was the most frequently used complementary
diagnostic test. Half of the patients were managed surgically: four in AO, and ten after
evacuation to NT, of which only one had surgery during deployment.
The increase in the number of discovertebral injuries together with the complexity
of their treatment shows the importance for Spanish Military Health System of
promoting and maintaining the Defense Central Hospital "Gómez Ulla" as fundamental
role in the medical support of international missions.
