Abstract
Introducción. El uso del linezolid está cada vez más extendido en el medio
hospitalario, incluyendo indicaciones o pautas posológicas distintas a las descritas en
ficha técnica, surgiendo la necesidad de estudiar este uso.
Material y método. Diseño: EUM indicación-precripción retrospectivo; Lugar y
periodo estudio: Hospital Central de la Defensa, 01 de septiembre al 30 de septiembre
de 2019; Objetivo: Conocer el uso de linezolid en el HCD “Gómez Ulla”. Muestreo y
tamaño muestral: Consecutivo no probabilístico, N=28. Elaboración ad hoc de CRD con
características clínico-demograficas y farmacológicas. Análisis estadístico descriptivo.
SPSS 20.0.
Resultados. El 17,8% de las indicaciones no estaban incluídas en ficha técnica,
correspondiendo a ITUc, IIAc y sindrome febril sin foco. La IPPB fue la indicación más
prevalente (53,5%) seguida de neumonía por microorganismos gram positivos (28,5%).
Destacar que el 82% de la muestra presentaba una creatinina en plasma ≥ 1,2mg/dl, y
que la infección se consideró nosocomial en 32% de los casos. La dosis utilizada fue
600 mg/12 h en todos los pacientes y la mediana de duración de tratamiento fue 9,5
días. La vía de administración intravenosa se utilizó en 71% de los casos, en 12,5 % no
se secuenció a la vía oral aunque no existía contraindicación para ello. No se detectaron
RAM con linezolid durante el estudio.
Conclusiones: No se observaron grandes desviaciones respecto a indicaciones y pauta
posológica de linezolid.
Se objetiva una posible intervención informativa respecto a la terapia secuencial de este
antibiótico mejorando su uso y la seguridad del paciente. Introduction: The use of Linezolide is spreading in the hospital melieu, including
indications or dosage regimen different from the ones described in the clinical summary
of product characteristics, suggesting the need to conduct this study.
Materials and methods: Design: DUS indication-prescription retrospective. Research
setting: Hospital Central de la Defensa Gómez Ulla (Madrid), September 1st September 30th 2019. Objective: To know the use of Linezolide at HCD Gómez Ulla.
Sampling and sample size: consecutive, non-probability sample; N=28. Ad hoc
development of CDR based on clinical-demographic and pharmacological traits.
Descriptive statistics. SPSS 20.0.
Results: 17,8% of indications were not gathered in the technical data sheet,
coresponding to UTIc, cIAI and undefined fever syndrome. SSTI was the most salient
indication of the research (53,5%) followed by pneumonia due to gram-positive
microorganisms (28,5 %). Two facts are to be highlighted: 82% of the sample presented
a level of creatinine in plasma of ≥ 1,2mg/dl, and in 32% of the cases the infection was
considered nosocomial. A dose of 600 mg/12 h was used with all the patients and the
average duration of the treatment was 9,5 days. IV administration was used in 71% of
the patients. After 72 hours, 12,5% of them showed no side-effects for PO although this
was not sequenced. No ADR with Linezolide was reported throughout the research.
Conclusions: No significant deviation from initial indications of Linezolide was
observed (endorsed by technical datasheet and/or scientific evidence).
A posible informative intervention in the sequential therapy of this specific antibiotic
was, however, observed, improving both its use and the patient’s safety.
