Abstract
El cáncer de próstata es actualmente, en los varones, la segunda neoplasia con mayor incidencia por detrás del cáncer de pulmón.
Prostatectomía radical y radioterapia son las dos técnicas más empleadas como tratamiento en la enfermedad localizada y localmente avanzada atendiendo a las recomendaciones de la EUA.
El objetivo principal de este trabajo es analizar si existen diferencias clínicas o en el tumor que condicionen la mayor utilización de un tratamiento frente a otro en un centro que no dispone de Radioterapia en su cartera de servicios. Además, hemos estudiado que técnica de las dos tiene más impacto en recaída de la enfermedad, metástasis o supervivencia.
Hemos encontrado que la edad y el tacto rectal han sido factores claves para indicar tratamiento con radioterapia, así como la cardiopatía isquémica. Los márgenes quirúrgicos afectados y el estadiaje de la pieza han sido los predictores más fuerte de recidiva bioquímica en el grupo de prostatectomía radical. Debemos interpretar con cuidado la mayor tasa de recidiva bioquímica en el grupo de prostatectomía radical. La evaluación de metástasis y muerte requiere de un mayor periodo de seguimiento en nuestra serie en ambos grupos de tratamiento.
Podemos concluir que, al margen de la edad, tacto rectal y presencia de cardiopatía isquémica, el hecho de no disponer de radioterapia en el centro estudiado condiciona su indicación y por tanto restringe las opciones terapéuticas entre nuestros pacientes. Prostate cancer is currently the second most common neoplasm in men after lung cancer.
Radical prostatectomy and radiotherapy are the two most used techniques for the treatment of localized and locally advanced disease in accordance with the recommendations of the EUA.
The main objective of this work is to analyze whether there are clinical or tumor differences that determine the greater use of one treatment versus the other in a center that does not have radiotherapy among its services. In addition, we have studied which of the two techniques has more impact on disease relapse, metastasis or survival.
We have found that age and rectal examination were key factors for indicating treatment with radiotherapy, as well as ischemic heart disease. Affected surgical margins and staging were the strongest predictors of biochemical recurrence in the radical prostatectomy group. The higher rate of biochemical recurrence in the radical prostatectomy group should be

interpreted with caution. The evaluation of metastasis and death requires a longer follow-up period in our series in both treatment groups.
We can conclude that, apart from age, rectal examination and the presence of ischemic heart disease, the fact that radiotherapy was not available in the center studied conditions its indication and therefore restricts the therapeutic options among our patients.
