dc.description.abstractIntroducción: La esclerosis múltiple se caracteriza por el efecto acumulativo de múltiples lesiones a diferentes niveles del sistema nervioso central, que varían en cada paciente y del momento evolutivo en el que se encuentren, y que derivan en distintos tipos con diferentes grados de discapacidad. De entre esta variedad de signos y síntomas, un gran porcentaje de pacientes con esclerosis múltiple presentan alteración de la marcha en el transcurso de la enfermedad. Estos síntomas son poco susceptibles de mejora mediante tratamiento farmacológico, sin embargo, muchos autores ponen énfasis en la terapia rehabilitadora, y espacialmente en la terapia física como principal vía en el tratamiento de dichas alteraciones. Para la recuperación de la marcha se propone la terapia mediante práctica imaginada con metrónomo. Dicha terapia consiste en la reproducción interna de un acto motor determinado, repetida extensamente con el objetivo de mejorar la ejecución del acto motor imaginado. Además se combina con el metrónomo para facilitar la imaginación motora y establecer así un ritmo de marcha. Objetivo: Elaborar una propuesta de intervención basada en la evidencia científica mediante el empleo de la imaginación motora en pacientes con esclerosis múltiple para la mejora de la marcha y documentar las mejoras en la marcha en pacientes con esclerosis múltiple tras la aplicación de un programa de imaginación motora combinada con tratamiento convencional. Metodología: Se realizó una Serie de Casos, con 6 casos de muestra, pertenecientes a la Asociación de Esclerosis Múltiple de Guadalajara. Cuatro de ellos estaban diagnosticados de esclerosis múltiple remitente-recurrente y dos de ellos de esclerosis múltiple progresiva secundaria. La edad varió entre los 31 y 54 años, formado por 3 hombres y 3 mujeres. Las variables medidas del estudio fueron fatiga, distancia recorrida en la marcha, y velocidad de la marcha. También se recogieron las sensaciones subjetivas de los pacientes. La terapia de práctica imaginada de la marcha con metrónomo se aplicó de manera intensiva durante 4 semanas, 6 días/semana, durante 15 minutos. Resultados: Se obtuvieron cambios en las tres variables del estudio, tanto en la fatiga, la distancia recorrida como en la velocidad de la marcha. Todos los pacientes tras la realización de la Escala Modificada de la Fatiga, obtuvieron valores por debajo de 38. Tras el test 6 minutos marcha se obtuvo una mejoría del 9.26% de media y tras el test de los 25 pasos se obtuvo una mejora de 15.6% de media. Conclusiones: La terapia de Práctica Imaginada de la marcha con metrónomo combinada con Fisioterapia Convencional es efectiva para la recuperación de la marcha en pacientes con esclerosis múltiple, remitente-recurrente y progresiva secundaria.es_ES
dc.description.abstractBackground: Multiple sclerosis, is characterized by the cumulative effect of multiple lesions at the different levels of the central nervous system. These lesions vary in each patient and it depend on the evolutionary moment in which they are found. They give rise to different types with different degrees of disability. Among this variety of signs and symptoms, a large percentage of patients with multiple sclerosis present altered gait in the course of the disease. These symptoms are unlikely to improve with pharmacological treatment, however, many authors emphasize rehabilitative therapy, and specially in physical therapy as the main route in the treatment of the alterations. Therapy through mental practice with metronome is proposed for the recovery of gait.Mental practise therapy consist of the internal reproduction of a certain motor act, repeated with the aim of improving the execution of the imagined motor act. In addition it is combined with the metronome to facilitate the motor imagery and thus establish a pace of gait. Objective: To elaborate a proposal of intervention based on the scientific evidence through the use of motor imagery in patients with multiple sclerosis for the improvement of the gait and to document the improvements in the gait in multiple sclerosis patients after the application of a motor imagery program combined with conventional treatment. Methods: A Series of Cases was carried out, with 6 sample cases, belonging to Multiple Sclerosis Association of Guadalajara. Four of them had been diagnosed with recurrent- remitting multiple sclerosis and two of them with secondary progressive multiple sclerosis. The age ranged between 31 to 54 years, with three men and three women. The variables measured in the study were fatigue, walking distance and walking speed. The subjective sensations of the patients were also collected. Imagery practice with metronome of gait was applied intensively for a period of 4 weeks, 6 days/week, for 15 minutes. Results: Changes were obtained in the three study variables: fatigue, walking distance and walking speed. After completing the modified scale of fatigue, all patients obtained values below 38.After the 6-Minute Walk Test, an average improvement of 9.26% was obtained and after the Timed 25-Foot Walk, an average improvement of 15.6% was obtained. Conclusions: The Mental Practice with metronome of gait combined with Conventional Physiotherapy is effective for the recovery of gait in patients with Multiple Sclerosis, relapsing-recurrent- remitting and secondary progressive.en
