Abstract
Las Enfermedades Cardiovasculares son, tanto a nivel nacional como internacional, una de las principales causas de muerte. En la actualidad y desde hace algunos años, se están desarrollando Programas de Rehabilitación Cardíaca basados en la realización de ejercicio físico. El Objetivo de este estudio ha sido describir los efectos del Programa de Rehabilitación Cardíaca, realizado en el Hospital Ramón y Cajal por pacientes que cursan con Enfermedades Cardiovasculares, sobre su estado físico y calidad de vida. Además, se comparó los resultados obtenidos con los de la bibliografía existente. Material y Métodos: se recogieron datos durante la Fase I y II del Protocolo a 20 pacientes, cuyo estado físico fue valorado previamente y a posteriori mediante una prueba de esfuerzo. Como complemento se analizaron también las siguientes variables: METs, frecuencia cardíaca, tensión arterial, peso, índice de masa corporal, perímetro abdominal, reducción funcional aeróbica y watts. Para evaluar la calidad de vida, se suministraron a los pacientes 3 cuestionarios psicológicos
(STAI Rasgo, STAI Estado y BECK) que fueron rellenados tanto al comenzar como al terminar el programa. El Programa consistió en la realización de ejercicio físico supervisado (como
mínimo 2 meses) en tapiz rodante o bicicleta estática, con una frecuencia de 3 veces por semana, controlando la frecuencia cardíaca, previa realización de una tabla de Fisioterapia.
Al acabar, aquellos que lo tuvieran programado, realizaban ejercicios de potenciación más intensos. Para la comparación con la bibliografía se realizó una búsqueda bibliográfica en las
bases de datos PubMed y Cochrane. Resultados: se ha obtenido una mejoría por parte de los pacientes en todas las variables estudiadas: en los METs (un 90% de los pacientes lo han mejorado), en la frecuencia cardíaca (50%), en la tensión arterial (70%), en el peso (60%), en el índice de masa corporal (70%), en el perímetro abdominal (85%), en la reducción funcional
aeróbica (70%) y en los watts (80%). En el caso de los test psicológicos la mejoría ha sido más ligera: un 40% para el STAI Rasgo y la escala BECK; y un 35% en el caso del STAI Estado.
Conclusión: la realización de Programas de Rehabilitación cardíaca en pacientes que cursen con Enfermedades Cardiovasculares, conlleva efectos beneficiosos para su estado físico y calidad de vida. Aún así, es necesaria una mayor labor de investigación encaminada a conocer los efectos de los Programas de Rehabilitación Cardíaca sobre pacientes de diferentes edades y distintos perfiles de riesgo cardíaco. Nowadays and for the last few years, Cardiac Rehabilitation Programs based on physical exercise are being developed. The aim of this study was to describe the effects of the Cardiac Rehabilitation Program, carried out at the Ramón y Cajal Hospital for patients with cardiovascular disease, on their physical condition and quality of life. In addition, we compared the results with those obtained in the existing bibliography. Material and Methods: Data was collected during the Phase I and II of Protocol to 20 patients, whose physical condition was analyzed before and after through a stress test. As a complement, the following variables were analyzed too: METs, heart rate, blood pressure, weight, body mass index, abdominal perimeter, functional reduction aerobic and watts. To assess the quality of life, the patients were provided with 3 psychological questionnaires (STAI trait, STAI State and BECK) which were filled at the beginning and at the end of the program. The program consisted of supervised physical exercise (2 months at least) on treadmill or stationary bike, 3 times a week, controlling the heart rate, holding a table of Physical Therapy. At the end, for those who had it in the schedule, strengthening exercise was intensified. To compare with the bibliography, a bibliographic search was made using PubMed and Cochrane data base. Results: There has been an improvement in the patients' condition in all the variables of the study: in the Mets (90% of the patients have improved), in heart rate (50 %), in blood pressure (70 %), in weight (60 %), in body mass index (70 %), abdominal perimeter (85 %), functional in the aerobic reduction (70 %) and watts (80 %). In the case of the psychological tests the improvement has been slighter: 40% for STAI trait and the scale BECK; and 35% in the case of the STAI State. Conclusion: the realization of Cardiac Rehabilitation Programs in patients with cardiovascular disease entails beneficial effects for their physical condition and quality of life. Even so, there must be a greater research work aimed at understanding the effects of Cardiac Rehabilitation Programs on patients of different ages and different profiles of cardiac risk.
